Gem of Seeing 5e
Wondrous item (adventuring gear), rare, requires attunement
This gem has 3 charges. As an action, you can speak the gem's command word and expend 1 charge. For the next 10 minutes, you have truesight out to 120 feet when you peer through the gem.
The gem regains 1d3 expended charges daily at dawn.

Are pesky illusions getting you down? Have you been swindled three times this week by fake jewelry? You need a Gem of Seeing!
A creature with truesight basically sees through illusions, normal and magical darkness, and shapechanging. It also sees onto the Ethereal Plane!
To roll a d3 without owning one, d6s can be easily broken down to have their results give out a d3 outcome with a 1 and 2 on the d6 be a 1, 3 and 4 a 2, and 5 and 6 a 3.
You do have to peer through the gem to get the truesight benefit. Consider attaching it to a pair of glasses or some other face fixture to keep your hands free
