Rod of Resurrection 5e
Rod, legendary (requires attunement by a cleric, druid, or paladin)
This rod has 5 charges. While you hold it, you can use an action to cast one of the following spells from it: Heal (expends 1 charge) or Resurrection (expends 5 charges.
The rod regains 1 expended charge daily at dawn. If the rod is reduced to 0 charges, roll a d20. On a 1, the rod disappears in a burst of radiance.

Death stops being a particularly large barrier for many upper-tier parties containing at least one cleric, druid, bard, or paladin. If you want to trivialize it further, a Rod of Resurrection can help you out.
You don’t need to expend the gold-costed components to cast Resurrection with the Rod.
Heal is a massive boost of hit points, and this can be 4 uses with no risk of destruction.
Casting Resurrection with the Rod runs the risk of destruction, but only a 5% chance. Most of the time, you’ll get a dozen or more uses before it breaks.
