Wondrous Item, very rare (requires attunement)
While wearing this amulet, you can use an action to name a location that you are familiar with on another plane of existence. Then make a DC 15 Intelligence check. On a successful check, you cast the Plane Shift spell. On a failure, you and each creature and object within 15 feet of you travel to a random destination. Roll a d100. On a 1–60, you travel to a random location on the plane you named. On a 61–100, you travel to a randomly determined plane of existence.

Want an at will “Get Out of Jail Free” card? Get an Amulet of the Planes! This handy dandy very rare item can give you a free Plane Shift daily that usually puts you close to where you want to be.
Plane Shift is cast with this item; if you cast it successfully, you and up to eight willing creatures still need to link hands to travel to another plane of existence.
If you fail the Intelligence check, you don’t cast the spell, and instead teleport you and every creature and object within 15 feet of you to either a random location on the plane you named or a randomly determined plane of existence.
Knowledge of teleportation circles can be beneficial, as it teleports you and the willing creatures to that exact circle on a successful check.
Plane Shift doesn’t just move you and your friends across planes; it also can banish creatures to other planes of existence, which you can choose should you successfully cast it with this item.
Once you start trying to use the item, you either cast it, or are teleported with everything around you somewhere random.
There aren’t limitations to this beyond getting lost; you can use this over and over again, teleporting all across space at a whim!
